All Climbers must fill in a customer registration and waiver form the first time they climb at any of X-Rock Climbing’s facilities. A Parent or Guardian must co-sign the wavier form for any person 18 years of age or under.

Coverage

You or your family will be compensated for death or bodily injury resulting solely and directly from an accident while climbing:

E     Death: US$10,000 or $20,000/person

E     Partial disablement: as per scale of permanent disablement up to US$10,000 or $20,000/pers.

E     Medical Expenses: actual medical expenses up to US$1,000 or $2,000/person

 While climbing at the X-Rock’s premise, valid on the day coverage is purchased.

 One minute to sign the request cover and give it to X-Rock’s officer before climbing

 -         intentional self injury or suicide or any attempt at suicide

-         participating in violence against one or more persons

-         you act criminally or illegally

-         you are under the influence of drugs or alcohol

-         as a result of insanity or pre-existing physical or mental defect or infirmity

 -         follow medical advise from a qualified medical practitioner as soon as possible

-         immediately contact X-Rock’s officer and declare your accident or claim declaration

-         obtain and provide all related medical documents and submit to X-Rock or direct to QBE for claim settlement
